If you have NEVER HAD a passport, you will need:Children under age 16 - Two parent consent and certified proof of parentage required:
- Certified copy (raised or colored seal) of your U.S. birth certificate issued by the state department of health. Hospital certificates are NOT acceptable. Must contain: date and place of birth, date of filing (within one year), name, registrar's seal and signature. Naturalized citizens must submit naturalization papers.
- Two 2x2 identical passport photos taken within the last 6 months (photos can be taken at this office for $12.50--cash or check).
- A valid driver's license (name must be identical to application; license must be valid for a minimum of two years) or someone with you with a valid driver's license who's known you at least 2 years.
- Check or money order ONLY to "U.S. Department of State" $75 for 16 & over $60 for 15 & under. (add $60 to this amount if you wish to expedite, add $14.85 if you would like return overnight delivery.)
- Know your parents' and most recent spouse's places and dates of birth.
- Know your social security number
- $25 cash or check to "Chittenden Superior Court."
- To appear at the Court in person (minors with parent or legal guardian).
- Sorry, we CANNOT accept credit or debit cards for payment.
- Must submit a certified copy of their birth certificate that indicates the name of both parents. (Note: if only one parent is listed on the birth certificate, only that parent need appear with the child.)
- Both parents must appear with valid ID and sign the application.
- If a 2nd parent is not available to appear:
- The appearing parent must present certified evidence that he/she has sole legal custody (birth certificate, divorce decree, adoption decree, death certificate)
Or- Notarized written consent of the other parent (include photocopy of parent's ID) for the issuance of a passport. (Consent forms are available at this office or through the link below.)
Or- In rare circumstances, a notarized written statement explaining why non-applying parent's consent cannot be obtained. (Use same consent form indicated above.)

If you have HAD a passport but cannot locate it you will need:
- The same documents listed above as well as a "lost passport" form if your old passport would still be VALID today.

If you STILL HAVE your old passport and you were under 16 when it was issued or it was issued 15 or more years ago:If you were over 16 when your passport was issued and that was less than 15 years ago:
Adults & minors 16-17: Submit your old passport instead of a birth certificate along with the same documents outlined above.
Children under 16: Submit your old passport and certified proof of parentage as indicated above. Two parent consent required.
